Proof of Vaccine Requests
The NHS App will be available to use as a vaccine passport from Monday 17 May 2021.
A paper version will also be available by calling 119.
You do not need to telephone your GP Practice to ask about the vaccine passport as they are unable to provide vaccine passports and it’s essential we leave phone lines free for people who need medical assistance.
The NHS App is separate to the NHS COVID-19 App, which is used for contact tracing.
You can already use the NHS App to book appointments, repeat prescriptions and see your full medical records.
Your vaccine passport will only show when you have had both doses.
If you don’t have the NHS App, download the NHS App now. It will record when you have had both doses of the vaccine and automatically create a vaccine passport which will be available from Monday 17 May 2021.
If you already have the NHS App, it will record when you have had both doses of the vaccine and automatically create a vaccine passport which will be available from Monday 17 May 2021. You do not need to do anything.
If you have the NHS App and get a message to say you need to contact your GP Practice, this is an extra option to get full access to your medical records in the NHS App. To get access to your full medical records in the NHS App, you need to contact your GP Practice and request access to your detailed coded record. Please avoid calling your GP practice at peak hours when people are trying to book appointments, and if possible email the team. The GP Practice will take appropriate time to action your request. This is completely separate from the vaccination information and will not affect your access to the vaccine passport.
If you are planning to travel, please register to use the NHS App at least two weeks before travelling.
The paper letter should be requested at least five days after a second vaccine, and can take up to five days to arrive so make sure to leave plenty of time if you need it to travel.

The surgery is now using a system called AccuRX it allows us to communicate with a patient regarding their health. If necessary we can send a message or document such as a sick note/med 3 to a patient or advice leaflets after consultations etc. One of the key features of AccuRX is the ability for a patient so send the surgery a message and photo related to their health issue.
We can also code any forms or questionnaires directly into the patient record so creating accurate and timely updates.
The Coppice Surgery/Angmering Medical Centre and Fitzalan Medical Group form a PCN, a PCN is a primary care network that consists of groups of general practices working together with a range of local providers, including across primary care, community services, social care and the voluntary sector, to offer more personalised, coordinated health and social care to their local populations. Networks would normally be based around natural local communities typically serving populations of at least 30,000 and not tending to exceed 50,000. They should be small enough to maintain the traditional strengths of general practice but at the same time large enough to provide resilience and support the development of integrated teams.
COPPICE AND ANGMERING MEDICAL CENTRE ARE OPERATING A TELEPHONE TRIAGE SYSTEM ONLY
In view of the developing circumstances surrounding the spread of Covid-19 and in line with NHS England policy, you will not be able to book routine or face to face appointments for the foreseeable future. All health concerns will be dealt with using our telephone triage system and we may offer video consultation.
Face to face appointments will only be offered after speaking to a clinician.

For any patients that do not at present have a nominated pharmacy, the
prescription will be sent to Kamsons Station Road, East Preston for you to collect until you nominate a pharmacy.

WEEKEND AND EVENING APPOINTMENTS
GP and nurse appointments are now available during the evening and at the weekend for people across Coastal West Sussex.
This means that urgent appointments are available up to 8pm during the week, and during clinics on a Saturday and Sunday.
This aims to improve access to GP appointments and help local people to get the help and support they need.
The service is run by Innovations in Primary Care (IPC), a not-for-profit company owned by local GP practices.
The appointments will be available at local GP access hubs, run from GP surgeries, and they will be both routine and urgent appointments with a doctor, nurse or healthcare professional.
The teams will see new illnesses when you urgently need to see a health professional, and offer planned appointments such as family planning and smear tests that can be hard to arrange whilst at work or when you are caring for family or a loved one.
IPC has been running the MIAMI clinics in Adur and Worthing for the last three years, which has offered a very similar service, offering help for ‘Minor Injury Assessment & Minor Illnesses’.
These clinics have been successful and helped local people to access the help they need outside of normal opening times.
We hope that this roll out of the hubs across the whole of Coastal West Sussex will mean that all patients registered with a GP practice in our area can benefit.
You book into the GP access hubs by ringing either the Coppice Surgery or Angmering Medical Centre and you may also be offered an appointment at one of the hubs at the weekend if you call NHS 111 for urgent health advice and support.
The hubs may be at your GP practice or they may be at a neighbouring GP practice locally in the community around you.
The teams will have access to your medical records with your consent, and any care they provide will be added to your notes so your GP practice can continue any ongoing care.
